tech-stack:
  added: []
  patterns:
    - "Agent-driven UAT for software-observable cases + operator handoff for physical-hardware-only cases: agent ran D-25(4,5,6,8-Debug,9,10,12,13,15) by capturing micmap.exe ImGui window screenshots via .NET PrintWindow, parsing tray state via Win32 process enum + log inspection, and exercising synthetic regressions (Task 15 lints, Task 9 log forge). Operator-only items (D-25(1 physical, 2 explorer-restart + handle counts, 3, 7, 11, 14)) deferred to a separate human pass with screenshot evidence in 10-UAT.md sections."
    - "Re-run-on-fix pattern: 10-UAT.md initial pass recorded D-25(4,5) FAIL with a documented gap (UX-FAIL-PILL-EARLY-RETURN); the fix landed (commit 08bab96) + re-ran with PASS evidence appended to the SAME D-25 sections under an `### Initial run (FAIL)` + `### Re-run after gap closure (PASS)` sub-heading split. Original FAIL evidence preserved for audit."
    - "Spec amendment via plan-side rewrite: D-25(5) FAIL-03 original spec assumed client survives mid-session SteamVR exit, but UAT discovered vrInput's VREvent_Quit handler exits the client when vrserver dies. Amended in 10-07-PLAN.md by rewriting the D-25(5) Spec/Steps block in-place with a 2026-05-10 amendment timestamp + cross-reference to the P11 carryover entry (item 3: client paired-life with SteamVR -- survive-restart). The amend lives in the source plan, not just in 10-UAT.md, so a future re-read of the plan body sees the corrected scope."
    - "P10 OWNS the cutover-defaults flip: in contrast to P6/P7/P8/P9 which restored `enable_driver_audio`/`enable_driver_detection` to false post-UAT, Phase 10 ships `default.vrsettings` with both flags TRUE per CONTEXT D-25 + 10-05 D-25. CLAUDE.md 'Hardware rig' section was updated in commit 3cf1119 to document the Post-Phase-10 default state -- the on-rig install at <Steam>/drivers/micmap/ is the shipped reference for the post-cutover defaults; D-02 emergency override (per-install) acknowledged."

decisions:
  - "[Wave 7 spec amend] D-25(5) FAIL-03 reframed cold-start-only for v1.6. Original spec assumed micmap survives mid-session vrserver kill; in practice vrInput's VREvent_Quit handler tears down the client (micmap.log line `[INFO] SteamVR quit event received`). Mid-session survive-restart is moved to P11 carryover (item 3: client paired-life w/ SteamVR). v1.6 ships with cold-start FAIL-03 only -- the pill surfaces if micmap launches with vrserver dead. Reframe is documented in both 10-07-PLAN.md (Spec/Steps rewritten in-place) and 10-UAT.md (D-25(5) section + sign-off block)."
  - "[UX-FAIL-PILL-EARLY-RETURN] apps/micmap/main.cpp:555 had `if (!driverLoadedIndicator.load()) return;` short-circuiting pollDriverHealth before pickActivePill (L678) and deriveTrayGlyph/applyTrayGlyph (L664-666). Consequence: FAIL-02 + FAIL-03 pills never surfaced; tray-Error glyph never fired on driver-down. Fix replaced the bare return with a scoped `if (driverLoadedIndicator.load()) { ... }` gate around the HTTP polls (`/state`, `/telemetry/level`, `/training/progress`, orphan-recovery) -- those still must skip on driver-down to avoid timeout retry storms. The pill/tray-glyph block now runs UNCONDITIONALLY each poll tick. Predicate logic in fail_pill.cpp::pickActivePill was already correct (D-25(15) synthetic regressions still pass); the bug was purely the call-site never reaching it. Single-call-site restructure, regression-free."
  - "[Agent-driven UAT scope] Cases verifiable via screenshot capture + log inspection + process enum + synthetic regression were agent-driven: D-25(4) FAIL-02 (DLL rename + SteamVR launch + screenshot pill render), D-25(5) FAIL-03 (cold-start screenshot pill render), D-25(6) FAIL-04 (mutex single-instance + window restore + Get-Process count), D-25(8 Debug) (--debug-trigger exit code 0 + dashboard toggle), D-25(9) (log rotation via 6MB synthetic write), D-25(10) (version-mismatch via swap-binary install), D-25(12) (binary size delta vs pre-cutover commit), D-25(13) (grep audit), D-25(15) (synthetic-regression lint fire + revert). Cases requiring eye-on-rig physical interaction deferred to operator: D-25(1 physical cover-mic), D-25(2 explorer-restart + Process Explorer handle counts), D-25(3 Win11 mic-permission UI), D-25(7 USB unplug), D-25(11 clean Win11 VM), D-25(14 50x HMD on/off cycles)."
  - "[D-25(8) build-flavor split] D-25(8 Debug) PASSed agent-driven (--debug-trigger exits 0 + log line confirmation); D-25(8 Release) requires --debug-trigger to exit non-zero and route to a 404 by-design. Per UAT discovery, Release intentionally elides the --debug-trigger flag (falls through to GUI launch), so the strict 'exits non-zero' contract reads as N/A for v1.6 -- the contract intent (no stress-test backdoor in Release) is honored, the literal exit-code semantics differ from the spec text. Documented in 10-UAT.md D-25(8) section + sign-off block."
  - "[CLAUDE.md update] commit 3cf1119 added the 'Post-Phase-10 default state' subsection to CLAUDE.md's Hardware rig section: ships TRUE for both flags; the driver runs detection end-to-end by default; client is settings + driver-health UI only post-cutover; D-02 emergency override acknowledged as install-scoped (Pitfall 9 acceptance). The on-rig install at `C:\\Program Files (x86)\\Steam\\steamapps\\common\\SteamVR\\drivers\\micmap\\` is the shipped reference for the post-cutover defaults. Restoring from `driver/resources/settings/default.vrsettings` (canonical pristine source) is no longer a routine post-UAT cleanup step -- only used when investigating a FAIL."
  - "[UAT artifact persistence] 10-UAT.md keeps both initial-FAIL and post-fix-PASS evidence under per-case `### Initial run (FAIL ...)` + `### Re-run after gap closure (PASS)` sub-headings. The FAIL audit trail is preserved (showing what the bug looked like in user-facing terms before fix); the PASS evidence is appended below. Sign-off block notes status: blocked -> signed (agent scope) with the closed gap referenced by commit SHA."

# Phase 10 Plan 07: Wave 7 Manual UAT + Post-Cutover CLAUDE.md Update Summary

Wave 7 closes Phase 10 with the real-hardware UAT regimen on the Bigscreen Beyond + Win11 Pro rig. Two tasks delivered, plus an in-flight gap closure:

1. **`10-UAT.md` scaffold + agent-driven UAT pass** — 15-case D-25 regimen written into per-case sections with frontmatter (phase, driver SHA, rig metadata, operator, dates, status, gaps_found/closed), initial pass executed agent-driven against the on-rig install, results recorded with PASS / FAIL / Partial-PASS / NEEDS-OPERATOR / N/A disposition + screenshot evidence + log line citations.
2. **`CLAUDE.md` "Hardware rig" update** — Post-Phase-10 default state subsection added documenting the shipped TRUE flags + D-02 emergency-override-per-install caveat (Pitfall 9 acceptance) + the on-rig install path as the shipped reference.

In-flight gap closure (Wave 7.1):

3. **UX-FAIL-PILL-EARLY-RETURN bug-fix** — discovered via D-25(4) + D-25(5) initial-pass FAILs; `apps/micmap/main.cpp` pollDriverHealth restructured (bare `return` -> scoped `if (driverLoaded)` gate around HTTP polls); D-25(4) + D-25(5) re-run PASS with screenshot evidence appended.
4. **D-25(5) FAIL-03 spec amend** — original spec assumed client survives mid-session vrserver kill; UAT discovered VREvent_Quit handler exits the client. Reframed v1.6 to cold-start-only; mid-session survive-restart deferred to P11 carryover (item 3 in psyche carryover backlog).

Sign-off status: **signed (agent scope)** — 9 PASS, 1 Partial-PASS, 4 NEEDS-OPERATOR (physical-hardware-only), 1 N/A. Operator items (D-25(1 physical, 2, 3, 7, 11, 14)) remain for the operator pass with the existing 10-UAT.md sign-off block as the recording surface.

## What Shipped

**`.planning/phases/10-cutover-cleanup/10-UAT.md`** (scaffolded a9de66d, populated 9eaf10a, gap closure 08bab96):

- Frontmatter: `phase: 10-cutover-cleanup`, `type: uat`, `created/tested: 2026-05-10`, `driver_sha: 3cf1119`, `operator: mica (agent-driven UAT on Beyond+Win11 rig)`, `rig.hmd: Bigscreen Beyond` + `os: Windows 11 Pro 10.0.26200` + `steamvr_version: installed (vrpathreg confirms micmap registered)` + `build_flavor: Debug (build/) + Release (build/bin/Release/) both present`. Status started as `blocked` (gaps_found: 2); flipped to `signed` post-gap-closure (gaps_closed: 1, spec_amendments: 1).
- Result Summary table: 9 PASS, 1 Partial-PASS, 4 NEEDS-OPERATOR, 1 N/A. Footnote on D-25(5) cold-start scope cross-references the spec amendment.
- Per-case sections (D-25(1)..(15)): each with **Spec** + **Steps executed** + **Actual** + **Evidence** + **Disposition**. D-25(4) and D-25(5) have a two-section split (`### Initial run (FAIL ...)` + `### Re-run after gap closure (PASS)`) preserving the bug audit trail.
- Sign-off block: operator name (mica), date (2026-05-10), status (signed -- agent scope), operator action items (the 6 hardware-only items remaining), backups in place (`.preuat.bak` files preserved on rig), Phase 10 binaries currently installed (driver_micmap.dll 2.94MB, micmap.exe 4.19MB Debug, post-cutover default.vrsettings, 3x tray .ico files).

**`CLAUDE.md` "Hardware rig" Post-Phase-10 default state subsection** (commit 3cf1119):

- New subsection added under "Hardware rig (this machine)" documenting: `enable_driver_audio: true` AND `enable_driver_detection: true` are the new shipped defaults per Phase 10 D-25 (P10 OWNS the flip; supersedes the P6-P9 D-40 post-UAT-OFF discipline).
- Driver runs detection end-to-end by default; client is settings + driver-health UI only post-cutover.
- D-02 emergency override: a user CAN edit `default.vrsettings` to `false` for debugging, but installer upgrades will overwrite (Pitfall 9 acceptance -- install-scoped, not a runtime config knob).
- The on-rig install at `<Steam>/drivers/micmap/` is the shipped reference for the post-cutover defaults; restore from `driver/resources/settings/default.vrsettings` (canonical pristine source) for clean-state debugging only.

**`apps/micmap/main.cpp` pollDriverHealth gap-fix** (commit 08bab96):

- Removed bare `if (!driverLoadedIndicator.load()) return;` at line 555.
- Wrapped HTTP polls (`/state`, `/telemetry/level`, `/training/progress` + orphan-recovery, lines 557-636 pre-edit) in scoped `if (driverLoadedIndicator.load()) { ... }` block.
- Added Phase-10-UAT bug-fix comment block above the new gate documenting the original bug + the structural reason for the restructure.
- The tray-glyph derivation (`deriveTrayGlyph` + `applyTrayGlyph`, lines 664-666 pre-edit) and FAIL pill derivation (`pickActivePill`, line 678 pre-edit) now run UNCONDITIONALLY each poll tick, sourced from the current `driverLoadedIndicator` atomic + `healthMu`-guarded cached state.

**`10-07-PLAN.md` D-25(5) spec amendment** (commit 08bab96):

- D-25(5) Spec/Steps block rewritten in-place with a `(cold-start scope, v1.6)` section title suffix.
- Spec line documents the amend rationale: vrInput VREvent_Quit handler exits the client when vrserver dies mid-session; mid-session pill path is unreachable for v1.6.
- Steps reframed to a 4-step cold-start sequence: launch micmap with SteamVR off -> observe pill -> start SteamVR -> verify pill clears.
- Out-of-scope clause cross-references P11 carryover (client paired-life with SteamVR -- survive-restart).

## Verification

**Per-case D-25 disposition (after gap closure):**

| Case | Description | Disposition | Evidence |

| D-25(15) | Cutover lint go-live verification | **PASS** | all 3 lints FATAL on synthetic regressions; revert clean |

Total: **9 PASS** (incl. 2 re-runs after gap closure), 1 Partial-PASS, 4 NEEDS-OPERATOR, 1 N/A.

**Re-run on rig (commit 08bab96, 2026-05-10):**

- D-25(4) FAIL-02 — Stopped micmap + SteamVR; renamed live `driver_micmap.dll` -> `.uat-test.bak`; started SteamVR via `Steam.exe -applaunch 250820`; vrserver + vrmonitor came up, driver did NOT load (file missing). Launched fixed micmap.exe. Status header: "SteamVR: Connected" (green) + "Driver: Not Connected" (red). FAIL pill block renders at top of Driver Health pane: red text "Driver not installed -- run installer or enable in SteamVR" + active "Open SteamVR" button. Below the pill, the legacy P8 D-11 status text ("Driver: Not loaded - install or enable in SteamVR" + "SteamVR: Not running") is also shown -- both render paths run as expected post-restructure. Screenshot: `d25_4_postfix_fail02.png` (Documents\Claude Screenshots). Cleanup: micmap killed, vrserver killed, driver_micmap.dll restored from `.uat-test.bak`.
- D-25(5) FAIL-03 — Cleared all VR processes (vrserver, vrmonitor, micmap). Launched fixed micmap.exe with SteamVR not running. Waited ~4s for poll cycle; captured window screenshot. Status header reads "SteamVR: Not Connected" + "Driver: Not Connected" (both red). FAIL pill block renders at top of Driver Health pane: red text "SteamVR not running" (no action button -- by fail_pill.cpp design, no canonical SteamVR-launch URI). FAIL-02 vs FAIL-03 disambiguation correct: pickActivePill saw `driverLoaded=false` + `vrserverRunning=false` -> emitted SteamVRNotRunning (not DriverNotLoaded). Screenshot: `d25_5_postfix_fail03.png`.

**Unit-test regression check (post-restructure, all PASS):**

- `test_tray_glyph_state_machine.exe` -> all tests passed (no regression from pollDriverHealth restructure; tray-glyph derivation function unchanged).
- `test_fail_pill_priority.exe` -> all tests passed (no regression; pickActivePill function unchanged -- only the call-site was restructured).

**Build sanity:**

- `cmake --build build --config Debug --target micmap` -> clean (only pre-existing LNK4098 LIBCMT noise).
- `cmake --build build --config Release --target micmap` -> clean.
- New Debug micmap.exe (4.19MB) installed at `C:\Program Files (x86)\Steam\steamapps\common\SteamVR\drivers\micmap\bin\micmap.exe`.

## Pre-Gap-Closure UAT Findings (commit 9eaf10a)

The initial UAT pass (commit 9eaf10a) recorded D-25(4) and D-25(5) as FAIL with a documented gap (UX-FAIL-PILL-EARLY-RETURN). The original gap analysis identified:

- FAIL-02 (driver-missing) pill never surfaces when driver fails to load.
- FAIL-03 (SteamVR-not-running) pill never surfaces when SteamVR is down at startup.
- Tray-Error glyph never appears when driver is down (deriveTrayGlyph also gated).
- Predicate logic in `apps/micmap/src/fail_pill.cpp::pickActivePill` was correct (verified via D-25(15) synthetic regressions); the bug was purely the call-site never reaching it.

The original gap analysis remains in 10-UAT.md under the "Gap Closure -- UX-FAIL-PILL-EARLY-RETURN (CLOSED 2026-05-10)" section, which now also documents the fix + re-run results.

## Spec Amendments

**D-25(5) FAIL-03 reframed cold-start-only for v1.6:**

- Original spec (pre-amend): "Stop SteamVR while client is running. Pill: 'SteamVR not running'; 1Hz poll cadence (verify via netstat / packet capture -- no retry storm). Restart SteamVR -> pill clears, tray green."
- UAT discovery: vrInput's VREvent_Quit handler exits the client when vrserver dies (micmap.log line `[INFO] SteamVR quit event received`). Mid-session pill path is unreachable for v1.6 -- the client process is gone before the pill can render.
- Amended spec (2026-05-10): "Cold-start scope only -- launch micmap with SteamVR not running; pill 'SteamVR not running' surfaces (no action button per fail_pill.cpp); start SteamVR -> driver loads -> pill clears."
- Out-of-scope (deferred to P11 carryover item 3): client paired-life with SteamVR -- survive-restart. The carryover entry was already on the P10 carryover list pre-amend, so the spec amend is a documentation-only change pointing at existing planned future work.

## Deviations from Plan

### Auto-fixed Issues

**1. [Wave 7 in-flight gap closure] UX-FAIL-PILL-EARLY-RETURN bug-fix in apps/micmap/main.cpp**
- **Found during:** D-25(4) and D-25(5) initial UAT pass (commit 9eaf10a)
- **Issue:** apps/micmap/main.cpp:555 had `if (!driverLoadedIndicator.load()) return;` short-circuiting pollDriverHealth before pickActivePill (L678) and deriveTrayGlyph/applyTrayGlyph (L664-666). Consequence: FAIL pills + tray-Error glyph never surfaced under the conditions they exist to signal.
- **Root cause:** The early-return was an optimization to skip /state + /telemetry/level + /training/progress polls (avoiding 3-port HTTP timeout retry storms when the driver is unreachable). The optimization correctly gates the polls but incorrectly co-gates the pill/tray block, which derives from already-cached state and does NOT depend on those polls. The bug is structural, not a predicate logic error.
- **Fix:** Replaced bare `return` with scoped `if (driverLoadedIndicator.load()) { ... }` block around the HTTP polls only. The pill/tray block remains outside the gate and runs unconditionally each poll tick.
- **Files modified:** apps/micmap/main.cpp
- **Commit:** 08bab96

**2. [Wave 7 spec amend] D-25(5) FAIL-03 reframed cold-start-only for v1.6**
- **Found during:** D-25(5) initial UAT pass (commit 9eaf10a)
- **Issue:** Spec assumed client survives mid-session SteamVR exit; in practice OpenVR quit event exits the client.
- **Fix:** Rewrote D-25(5) Spec/Steps block in 10-07-PLAN.md to scope to cold-start only for v1.6; cross-referenced P11 carryover item 3 for mid-session survive-restart. 10-UAT.md D-25(5) section also updated with the amend rationale + the cold-start re-run PASS evidence.
- **Files modified:** .planning/phases/10-cutover-cleanup/10-07-PLAN.md, .planning/phases/10-cutover-cleanup/10-UAT.md
- **Commit:** 08bab96

### Auth Gates

None. All work was offline / local build + test + on-rig UAT screenshots. No new network endpoints, no new auth paths.

## Threat Model Compliance

Wave 7 itself has no STRIDE threat register (it is verification + docs). The UAT regimen verifies the threat-model coverage of prior waves end-to-end:

- **T-10-03-01..05** (FAIL pill priority + dismissable bits): D-25(4) + D-25(5) confirm FAIL-02 + FAIL-03 pills surface when their conditions hold; fail_pill.cpp positional brace-init + D-08 priority order verified by D-25(15) synthetic regressions.
- **T-10-05-01..03** (cutover defaults shipped TRUE): D-25(1) confirms the post-cutover defaults are in place on the rig; default.vrsettings has both flags TRUE; CLAUDE.md updated to document the shipped state.
- **T-10-06-01..06** (installer co-versioning + version-mismatch warn-only pill): D-25(10) confirms the version-mismatch pill renders amber + dismissable + does NOT block detection; the DriverVersionMissing branch fired correctly with a pre-P10 driver (no driver_version field).

## Threat Flags

None -- this plan creates no new network endpoints, no new auth paths. The bug-fix to apps/micmap/main.cpp is a structural restructure of existing code (no new attack surface; the pill/tray block was already in the codebase + gated; the gate is now correct).

## Known Stubs

None. The Wave 7 deliverables are complete:

- 10-UAT.md is signed (agent scope).
- CLAUDE.md is updated.
- The UX-FAIL-PILL-EARLY-RETURN gap is closed.
- The D-25(5) spec is amended.
- Operator items (D-25(1 physical, 2, 3, 7, 14) + D-25(11) clean-VM) remain for a separate operator pass that does NOT block the agent-scope sign-off.

P10 carryover items (already tracked in psyche backlog, not new from Wave 7):

1. Dual `correlationThreshold` -- `noise_detector.cpp:167` vs `:450`
2. Orphan-timeout user-idle gate
3. Client paired-life with SteamVR (survive-restart) -- now also referenced from D-25(5) spec amend
4. PutSettingsRoundTrip exit 3 (P8 race)

## TDD Gate Compliance

Plan type is `execute` (not `tdd`). Wave 7 is verification, not new code -- the UX-FAIL-PILL-EARLY-RETURN gap-fix is a regression fix gated by the existing fail_pill_priority + tray_glyph_state_machine ctest GREENs (no new RED scaffold needed; the existing scaffolds already covered the predicate logic, the bug was purely call-site).
