test(attach): give each advisory arm its own effect seq

Every arm of the client-policy case reused one Rc op seq, so each arm after the first replayed the journal instead of opening a stream and was handed back an id the previous arm's opener exit had terminal-retired. The failure read as a defect in the policy record the case exists to check, and it was a reused integer: an identity-free arm reds in second position exactly as hard as one carrying a stale identity. Give each arm a distinct seq, run the identity-free control both first and second so a lifecycle defect can no longer read as a claim about declared identity, and name the arm in every assertion, panic and open/close line so a failure can attribute itself. The mechanism is recorded on the helper, where the next reader meets it before re-deriving it. Not yet executed; the negative control is on file as two reds with two different error strings.
